The following documents are REQUIRED to be uploaded as part of the submission.
- Certificate of General Liability Insurance
- A declaration page is not an acceptable form of proof of insurance
- Renew Financial does not have to be listed as a certificate holder
- California – minimum policy requirement $1M
- Florida – minimum policy requirement $500k
- A declaration page is not an acceptable form of proof of insurance
- Certificate of Workers Compensation Insurance
- California – If exempt, no documentation needed
- Florida – If exempt, the Workers Compensation Exemption Letter is required
- Photo of Driver’s License for Principal Owner/RMO and Qualifier (if applicable). Please note that they will be required to sign our Participation Agreement.
